Official Sweepstakes Rules — Maxivita Apple Watch Ultra Giveaway

I. Sweepstakes Period

The “Maxivita Apple Watch Ultra Giveaway” consists of recurring monthly sweepstakes. Each sweepstakes begins at 12:00:01 a.m. Central Time on the first day of each calendar month and ends at 11:59:59 p.m. Central Time on the last day of that same calendar month (each a “Promotion Period”). Each Promotion Period is a separate Sweepstakes. Entries do not carry over into subsequent months. Sponsor’s computer is the official time‑keeping device for determining the timing of entries.

II. Sponsor

EG Sales LLC d/b/a Maxivita (the “Sponsor”), 2609 Technology Dr., Plano, TX 75074.

III. Eligibility

Open only to legal residents of the fifty (50) United States and the District of Columbia who are eighteen (18) years of age or older at time of entry. Employees, officers, directors, representatives and agents of Sponsor and its affiliates, and members of their immediate family/household, are not eligible. Void where prohibited by law.

IV. How to Enter

1. Online (Free): Submit the entry form on this page during the Promotion Period.
2. Automatic Entry with Purchase: Completing a purchase on maxivitaco.com during the Promotion Period automatically grants one (1) entry using the billing name and email associated with that order. No purchase is required to enter or win and a purchase does not increase your chances of winning.
3. Mail‑In (No Purchase Necessary): On a 3" x 5" card, handwrite “Maxivita Apple Watch Ultra Giveaway,” your full name, mailing address, city, state, ZIP code, email address, and date of birth. Mail in a stamped envelope to: Giveaway Entry — Maxivita, 2609 Technology Dr., Plano, TX 75074. Mail‑in entries must be postmarked by the End Date and received within 5 days thereafter.

Limit one (1) total entry per person/household/email address, regardless of method. Bulk entries and entries in excess of the limit will be void. No purchase or payment is necessary to enter or win. A purchase will not increase your chances of winning.

V. Prize

One (1) winner will receive one (1) Apple Watch Ultra (model subject to availability). Approximate Retail Value (“ARV”): $799. Odds of winning depend on the number of eligible entries received. Prize is non‑transferable; no cash or substitution except by Sponsor at its sole discretion if prize becomes unavailable.

VI. Winner Selection & Notification

Potential winner will be selected in a random drawing from among all eligible entries received during the Promotion Period, to be conducted by Sponsor within seven (7) days after the End Date. Potential winner will be notified by email (and/or SMS if provided) and must respond within seventy‑two (72) hours with eligibility confirmation and any required affidavits/releases. If potential winner cannot be contacted, is ineligible, or fails to respond on time, an alternate may be selected.

VII. General Conditions

Sponsor reserves the right, in its sole discretion, to cancel, modify, or suspend the Sweepstakes should fraud, technical failures, or any other factor beyond Sponsor’s control impair the integrity or proper functioning of the Sweepstakes, as determined by Sponsor. Sponsor reserves the right to disqualify any individual it finds to be tampering with the entry process or the operation of the Sweepstakes or to be acting in violation of these Rules or in an unsportsmanlike or disruptive manner.

Sponsor’s Right to Modify: Sponsor reserves the right to modify these Official Rules or the Sweepstakes (or any portion thereof) at any time, including to comply with applicable law or address circumstances affecting the Sweepstakes’ integrity or operation, without prior notice.

VIII. Release & Limitations of Liability

By participating, entrants agree to release and hold harmless Sponsor, its parents, subsidiaries, affiliates, advertising and promotion agencies, and each of their respective officers, directors, employees, and agents (collectively, “Released Parties”) from any claims, liability, loss, injury, or damage arising from participation in the Sweepstakes or the acceptance, receipt, use, or misuse of any prize.

IX. Publicity

Except where prohibited, participation constitutes consent for Sponsor to use entrant’s name, city/state, likeness and entry for promotional purposes in any media without additional compensation or notice.

X. Disputes/Choice of Law

To the fullest extent permitted by law, entrants agree that any and all disputes, claims, and causes of action arising out of or connected with this Sweepstakes or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the state or federal courts located in Collin County, Texas. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules shall be governed by the laws of the State of Texas, without giving effect to any choice of law or conflict of law rules.

XI. Privacy

Information collected from entrants is subject to Sponsor’s Privacy Policy. By entering, you consent to Sponsor’s collection, use, and disclosure of your personal information for the administration of the Sweepstakes and as otherwise described in the Privacy Policy.

XII. Winner List

For the name of the winner, send an email request to support@maxivitaco.com with the subject line “Maxivita Apple Watch Winner List” within thirty (30) days after the End Date, or mail a self‑addressed, stamped envelope to the Sponsor address above.

XIII. Third‑Party Notices

Apple, Inc. is not a sponsor of, nor affiliated with, this Sweepstakes. Meta, Instagram, TikTok, X/Twitter, Google, and Shopify are not associated with or responsible for this promotion. Any trademarks used are the property of their respective owners.

XIV. Taxes

Winner is solely responsible for any and all federal, state, and local taxes and any other costs and expenses associated with prize acceptance and use not specified herein as being provided.
